Alyssa Gutierrez is in graduate school for occupational therapy after discovering her passion for helping children and adults develop their motor skills through her hands-on experiences in the Exercise Science program at PETM. “I enjoy the gratitude I get helping people become more successful with daily activities. FORMS FOR PETM STUDENTS
Complete the PETM Admission Application, if you are a current PETM student and want to add a PE/TCEM minor or certificate to your current program. If you are not a current PETM student and want to be admitted to PETM, click here.
Complete the change of major form, if you are changing from one PETM major/certificate to another.
Complete the double major form, if you are planning to pursue multiple majors/certificates in PETM.
Application Deadlines
- May 1 for December graduates
- October 1 for May graduates
- February 1 for August graduates
A dismissed PETM student who wishes to be reinstated must complete the Reinstatement Application by the following deadlines:
- June 15 for fall semester
- October 15 for spring semester
- March 15 for summer session
